Since Grand Peaks is a non-profit Community Health Center, they receive some funding to help support their services and allow them to offer discounts. All patients wishing to receive a discount must complete the Sliding Scale application and provide proof of household income. Whether the patient is insured or not, if the patient qualifies for the Sliding Scale, the patient balance can be discounted. The amount of discount depends on the sliding scale applied to the account after application is considered. Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical center in 2007, with one Provider and a small staff.  The Dental center was added in 2010.  Have now grown to serve patients in St. Anthony, Rexburg and surrounding areas of the Upper Valley.
